We have provided your new patient forms for you online. Please complete and submit them prior to meeting with Dr. Keith Warr or Dr. Margaret Hyams. Doing so will save you time when you arrive at Warr Dental and streamline your experience in our office. If you’re unable to fill out the forms before your appointment, no problem! We have forms available in the office that you can fill out upon arrival.
